WORKERS’ COMPENSATION – DESIGNATED PROVIDER PROGRAM
Employment Solutions works to provide a safe working environment for all employees. However, injuries on the job do occur, and we have arranged for prompt medical treatment of these injuries through a designated provider. If you are injured at work, the only authorized facilities for treatment are the locations listed below.
IMPORTANT NOTE
ANY MEDICAL COST OTHER THAN THAT THROUGH THE DESIGNATED PROVIDER OR ITS REFERRALS WILL BE THE RESPONSIBILITY OF THE EMPLOYEE.
IF YOU ARE INJURED AND REQUIRE MEDICAL CARE
Treatment should be obtained from the nearest emergency medical facility (hospital E.R.). Notify both your on-site supervisor and Employment Solutions as soon as possible if this occurs.
